Jami co-chairs the planning and implementation of a statewide learning collaborative under the Regional Health Connector Program at Trailhead Institute. This collaborative focuses on systems changes to expand partnerships between clinical settings and community resources aimed at preventing, detecting, and controlling cardiovascular disease.
Prior to joining Trailhead Institute, Jami served as the Health Disparities and Community Grant Program Manager in the Office of Health Equity at the Colorado Department of Public Health and Environment. In that role, she developed the program’s vision to address health inequities by funding community-led systems and policy changes that focused on the root causes of poor health outcomes.
Throughout her career, Jami has held various positions in nonprofit organizations and higher education. Her values align with the mission of The Civic Canopy to foster connections and relationships that create conditions for communities to thrive. She is particularly inspired by her work with grassroots organizations and the development of trusting relationships.
